How much of the NREMT is operations?

About one question in eight to ten, mostly rules you can learn once and keep.

What does operations cover?

Everything around the patient.

  1. Legal and ethical issues

Consent (expressed, implied, minors), refusal, DNR orders, abandonment, negligence, confidentiality, mandatory reporting.

  2. Documentation and communication

Objective patient care reports, correcting errors, radio reports and hand-offs.

  3. Mass-casualty incidents

    generated

Incident command, START and JumpSTART triage. Our mocks generate START questions with the category checked by code.

  4. Ambulance operations

Driving safety, lights and sirens, cleaning and restocking, air medical landing zones.

  5. Responder wellness

Lifting technique, stress, fatigue and getting help.

What do operations questions look like?

Rules about consent, records and triage. The first two are worked.

  1. NREMT-style question 1 · Triage

Using START, a patient who cannot walk breathes 36 times a minute, has a radial pulse and follows commands. Which category?

    1. AImmediate (red)
    2. BDelayed (yellow)
    3. CExpectant (black)
    4. DMinor (green)
    Worked answer

A: Immediate (red). Breathing over 30 a minute makes a patient immediate in START, whatever else is normal.

The trap: Seeing the normal pulse and mental status and choosing delayed.

  2. NREMT-style question 2 · Legal

An unresponsive adult with no family present needs care. What type of consent applies?

    1. AInformed refusal
    2. BImplied consent
    3. CExpressed consent
    4. DNo care is allowed
    Worked answer

B: Implied consent. An unresponsive patient is assumed to want life-saving care.

  3. NREMT-style question 3 · Documentation

You find an error in a patient care report you already submitted. What should you do?

    1. AAsk your partner to rewrite it
    2. BDelete the original
    3. CFile a correction per policy, noting the date and time
    4. DLeave it alone
    Show the answer

C: File a correction per policy, noting the date and time. Records are corrected openly, never erased.

  4. NREMT-style question 4 · Wellness

    Which lifting technique is safest?

    1. ABend at the waist and lift fast
    2. BLift with the arms held straight out
    3. CTwist while lifting to save time
    4. DKeep the weight close, back straight, lift with the legs
    Show the answer

D: Keep the weight close, back straight, lift with the legs. A power lift keeps the load close and uses the legs.

How does START triage work?

A 60-second sort by walking, breathing, perfusion and mental status.

The START algorithm for adults, as taught in EMT courses (JumpSTART for children differs)

What rules win operations questions?

Four rules.

  1. Competent adults decide

They may refuse. Inform, encourage, document.

  2. Write what you saw

    Objective, timed, corrected openly.

  3. Triage sorts, it does not treat

Only open airways and control major bleeding during START.

  4. You matter too

Safe lifting, safe driving and asking for help with stress are part of the job.

What traps are set in operations questions?

Rules bent for good reasons.

  • Transporting a competent adult who refuses.
  • Honoring a DNR that cannot be produced or verified.
  • Starting CPR during START triage.
  • Deleting an error in a report.
  • Driving with lights and sirens as if they gave you right of way.

How do I study operations?

Three short sessions.

Our plan, with minutes a day

Frequently asked questions

Is triage on the NREMT?

Yes. Mass-casualty incidents and triage are part of the operations domain.

What START category is a patient breathing over 30 times a minute?

Immediate (red).

Can a competent adult refuse EMS care?

Yes. Make sure they understand the risks, encourage care, document the refusal and advise them to call back.

Do these questions follow my local protocols?

They follow national EMT education standards, which is what the exam tests. Your local protocols and medical direction govern real patient care.
